I carry one of these: http://www.parktool.com/products/detail.asp?cat=7&item=MTB-3
Missing link,
spare tube,
Swiss army knife,
a spare hanger.
And food.
that can get me out of just about anything.
on longer rides I carry a first aid kit and emergency blanket. Plus a cellphone.

I carry one of these http://www.topeak.com/products/Tools/Hexus16blackW_O_Bag
Almost half the weight, probably a quater of the price of things like that park tool, or the alien series. Yet has pretty much everything I need to fix stuff on my bike. Only thing missing is 8mm and 10mm spanners...

patchkit,
sparetube (well patched)
spaceblanket,
crankbro's 5 tool (enough hex's and a phillips)
park chaintool
park triangle spoketool
pump
quicklink
that about covers it i think
oh yeah, One Square Meal bar or two, or half (if i've nibbled), often a light, and pretty much always a cellphone. (went out without one recently at night on my own and felt decidedly unsafe... weird considering not 5 or 8 years ago wouldnt even have crossed my mind...)

Crank Bros multi-tool with chain breaker (Ok but not a great tool)
Tyre levers
Tube
Piece of old road tyre to use as a "boot" to do temp repairs on gashed tyres
Derailleur hanger (used twice in the last 6 months but on other peoples bikes thankfully)
Zip ties (millions of uses)
Puncture repair kit
Pump
SRAM powerlink
OSM
Phone
All of the above used to fix my bike or others in the last 12 months

Pretty much the same stuff when out for a mountain bike ride too.
Trouble I have with the Parktools though, is that they seem to have not so good edges on their hex keys.

I carry a pocket chainsaw and a folding saw on rides that I know there is likely to be trees across the track. I dont carry these on my normal rides up the port hills tho...
